How can I obtain a copy of the death certificate?
The death certificate is completed in two sections: the medical portion is completed by the KCMEO, while the remainder is completed by the funeral home. Once the death certificate is completed, it is the responsibility of the funeral home to file the document with the WI State Office of Vital Statistics. The funeral home can then provide you with copies of the death certificate.
