Application and Requirements
Application and fee must be submitted at least 10 days prior to an event:
Temporary Food Service Fee Schedule (July 1, 2019 - June 30, 2020)
- Temporary Restaurant/Retail Fee:
- $152.00 - 1st Event during April 1 - June 30 time period.
- $30.00- Each subsequent event after paying initial $152.00

If you have any questions, contact a health inspector at 262-605-6700, Monday through Friday between the hours of 8:00 AM and 5:00 PM.
Payments
Fee can be paid with cash, money order, local check or
credit card. Money orders and local checks should be made payable to the: Kenosha County Division of Health.
