Does this cost anything? The CCS is program is covered by Medicaid/Forward Health, state insurance. Unless you are determined to have a cost share under Medicaid, there is little to no cost to you. You would still be responsible for any out of pocket costs to pay for your Medicaid/Forward Health premiums. If you do not have Medicaid, the resource center can help determine how we can support you getting connected. Will my information get back to the courts/probation or parole officers/DCFS workers? The CCS program abides by all privacy laws and information sharing is treated within the confines of the law.
What is case management? Case management is a task by a service facilitator that helps you with the process of planning, facilitation and collaboration by the service facilitator who advocates for the participant to be enrolled in resources and supports to meet their identified needs, through assessment and communication. The service facilitator helps locate identified supports and funds them.
What can the service facilitator help me with? The service facilitator can help with your identified goals and interventions identified through the assessment and service plan. This may include support in connecting to a therapist, providers, medication management, or individual skills worker. The service facilitator can also help support the participant with individual goals such as: maintaining mental and physical health, utilizing healthy coping skills, medication compliance, obtaining healthy support systems and/or other identified goals relating to mental health stability and community living.
Will the service facilitator find me a place to live? No. The service facilitator will assist the participant in connecting to resources that support the participant’s housing needs i.e.: help signing up for section 8 voucher and/or provide information about potential apartments in the consumers price range.
Do I have to participate? Is it mandatory? No. The CCS program is a voluntary program and participation is solely at the discretion of the participant. Participants who are enrolled into CCS, should be willing to engage in services to help meet their identified needs.
